The 4-Step Admission Blueprint
The entire visit typically takes between two to three hours. We don’t rush. We stay until every question is answered and you feel completely safe.
1.The Clinical Assessment &Consent:First 45 Minutes.
The nurse performs a comprehensive physical assessment of your loved one, reviewing their current vitals, symptom levels, and medical history. Simultaneously, we review and sign the standard consent forms. This paperwork formally transitions the care focus from curative treatments to comfort-oriented care under the Medicare Hospice Benefit.
2.The Medication & Equipment Audit:
Next 30 Minutes.
We go through every bottle in the house. The nurse will determine which curative medications are no longer helping and which new comfort medications need to be ordered. We will also coordinate the delivery of medical equipment—like a hospital bed, oxygen concentrator, wheelchair, or specialized mattress—delivered straight to your home at no charge.
3.Building Your Custom Care Plan:
Next 30 Minutes.
Hospice is a team sport. We map out a customized visitation schedule based on your specific needs. 4.Emergency & Crisis Education:
